Where can I obtain a copy of the Authorization Release Form?

An Authorization Release Form can be found in Section VIII, Page 13 of this manual. Copies may be made or you may pick up additional copies at the Customer Service Center which is located in the Business Development and Permit Center, 3300 Central Parkway, Cincinnati, Ohio 45225.

When do I need to Submit an Authorization Release Form?

This form is used if you are not the owner of the project. Following are some scenarios in which you are required to submit an Authorization Release Form:

  • If you want to withdraw a project from the City on behalf of the owner.

  • If you want to remove the plans for a project from City premises on behalf of the owner.

How do I submit an Authorization Release Form?

To submit an Authorization Release Form, you simply fill in the information about your project on the application and submit to the Customer Service Center located at the Business Development and Permit Center, 3300 Central Parkway, Cincinnati, Ohio 45225. The Center is open Monday - Friday, 8:00am - 5:00pm

What happens to my Authorization Release Form after I submit it?

After submission, the information on your application is entered into a database for tracking purposes and copies of the plans you provided are removed from the process and returned to the applicant.
